How to Place Your Order:
Please fill out one form per dancer, per show. If your dancer is performing in multiple shows and you would like them to receive a gift at each, you must submit a separate form for each show. Be sure to select only one showtime per order form.

Example:
Emma is performing in the 2:00pm and 6:00pm shows. If she should receive a gift at both, a separate form must be submitted for each showtime.
